    Appointment Process

    Thank you for your interest in our practice at the Vasectomy Specialist Clinics of Chicago where we perform No SNIP Vasectomy.
    At present, during the pandemic, we are operating at our Oak Brook office only (600 Enterprise Dr., Suite 218, Oak Brook, IL 60523).

    APPOINTMENT OPENINGS:

    We may have a few openings available for consultation on the upcoming Monday or Tuesday between 10:00 AM to 3:00 PM. (Except holidays)
    If these days do not work for you or you wish to schedule later, please call, or email your desired date for consultation.

    THE PROCESS:

    The process of having a vasectomy involves the following.
    1. Zoom video consultation in which Dr. Agha will explain to you the procedure.
    2. The actual procedure will be scheduled in the office on Thursdays or Fridays after consultation.
    3. You will be required to have a COVID-19 test 48 hours before the procedure. Our office will make arrangements for you to have this test at a lab.

    COVERAGE:

    We accept almost all PPO commercial insurances. We do not accept HMOs.
    Prior to scheduling your procedure, you are advised to call your insurance company for coverage. They may ask you various questions. Please let them know that the Procedure Code for the Vasectomy is 55250 and the Diagnosis Code is Z30.2 Additional incidental codes may be applied. The procedure is done in the office. For in-network benefits, let them know that Dr. Agha will perform the procedure under Vasectomy Specialist Clinics of Chicago. You may want to ask your insurance company about your co-pay for the office visit and for the surgery and if you have met your deductibles. Please get a reference number or case number of your conversation.

    For self-pay patients, the total cost is $1700.00 (It includes Consult $250, Procedure $1250, Follow up visit $100 and Mandatory Pathology report by a pathologist is $100)

    You can choose to have the procedure either under the insurance or under the self-pay package.
